1. Claim forms

In case of death caused by illness
  1. Completed death claim form (Click Here to Download )
  2. Life insurance certificate (original copy)
  3. Certified copies of secondary beneficiary’s ID card and family book along with the original copies
  4. Certified copy of death certificate along with the original copy
  5. Consent letter of secondary beneficiary(ies) or heir(s) to disclose personal information
  6. Medical report from doctor in case of death in the registered hospital or clinic
  7. Other documents as deemed necessary by the Company to replace or support any of the documents mentioned above
In case of death caused by accident

In this case, the documents for the case of death due to illness are required the same, including the two additional documents below

  1. Completed death caused by accident (Click Here to Download )
  2. Copy of accident report which is certified by investigator (police)
  3. Certified copy of autopsy examination report as deemed necessary by the Company to replace or support any of the documents mentioned above
In case of Total Permanent Disability (TPD)
  1. Completed Claim Request Form of Total Permanent Disability (TPD) (Click Here to Download )
  2. Life Insurance Certificate (original copy)
  3. Certified copy of insured’s ID card along with the original copy
  4. Medical report as determined by the Company
  5. Other documents as deemed necessary by the Company to replace or support any of the documents mentioned above

2. How to submit claim request

Upon the insured’s death or total permanent disability (TPD), the beneficiary(ies), the insured, or legal representative can submit claim request to Sovannaphum Life by choosing one of the easy methods below:

  1. Company’s Head Office at Canadia Tower, No. 315, 25th Floor, Preah Ang Duong St. (110), corner Preah Monivong Blvd (93), Phnom Penh, or the Company’s branch, or insurance sale staff at bank partners or
